VISION: As a group, Latinos have disproportionately higher rates of certain chronic conditions. Our vision is to eliminate the disparities that exist between ethnic groups and to reduce the chronic disease prevalence in the Latino community.
BARRIERS: Community-based, integrated chronic care systems offer great potential for developing culturally proficient screening, assessment, treatment and rehabilitative services for Latino communities. The core goals for such integrated care systems include accessibility, affordability and acceptability/cultural proficiency. CHARGE: To develop a strategic blueprint for creating a culturally proficient, integrated service delivery network capable of providing disease management services for Latino communities. |
