Outreach Services

San Ysidro Health is proud to provide outreach and education services in the community.

Outreach services help to facilitate access to quality healthcare and address social needs of patients outside of traditional care settings. Through our community outreach efforts, San Ysidro Health brings critical services and information directly to patients and others in convenient community locations.

Below are the types of community outreach available through our programs.

If you’re interested in requesting a booth at a community event, please contact the San Ysidro Health External Affairs Department at SYH-Ext-Outreach@syhealth.org at least four weeks in advance of your event.

Health Insurance Outreach Services:

The Health Insurance Outreach Team educates the community about affordable health insurance options and financial assistance available.

San Ysidro Health conducts outreach at various community locations and is available to give presentations to organizations and community groups that provide a simple explanation of free and low-cost health insurance options. The Health Insurance Outreach team is also available for table outreach opportunities at health fairs and events.

In addition, San Ysidro Health has a Health Insurance Hotline available at (619) 600-3000. This hotline provides phone screening and health insurance enrollment appointments to those that qualify for health insurance plans with Covered California and Medi-Cal. Health Promotion Outreach Services:

The Health Promotion Team provides informational health educational materials on a variety of topics including chronic disease management (e.g., diabetes, hypertension, high cholesterol, cancer, etc.), healthy lifestyles modifications and prevention, and nutrition and physical activity. The team provides services at clinic sites, schools, churches, community centers, grocery stores, malls, street outreach, community health fairs, and more.

HIV Outreach Services:

The HIV Department provides a comprehensive array of quality HIV services in a caring and supportive environment. Our services include HIV testing, medical services, and social support for infected and affected individuals and their families.  Outreach and HIV testing may be conducted at clinic sites, schools, churches, community centers, grocery stores, malls, street outreach, community health fairs, and others. Dental Department Outreach Services:

The Dental Department Outreach Team provides school-based dental Screenings for children 1 to 15 years of age, addressing oral hygiene and fluoride application. Outreach efforts identify students with high risk of caries and provide education for school-based events for parents and caregivers

Mobile Health Outreach Services:

The Mobile Health Services Team provides pediatric preventive health care, school and sports physicals, immunizations, WIC Hemoglobin test, and minor sick visits. Blood Pressure and Body Mass Index Screenings are also available at health fairs depending on staff availability and other community events. Areas of service include shopping malls, public housing, retail stores, schools, parks, and recreation centers that can accommodate safe operation of the mobile unit.
