San Ysidro Health is proud to provide Outreach and Education services in the community.

Health Insurance Outreach & Education Services:

The Health Insurance Outreach Team educates the community and general public about the new affordable health insurance options and financial assistance available under the new health law called the Affordable Care Act (ACA).

Covered California has a set open enrollment period, but people with major life events can apply after the enrollment period closes, if within 60 days of said event.

There is no deadline for Medi-Cal enrollment.

This Team can also provide information about San Ysidro Health programs and services during outreach.

Effective in January 2014, all U.S. citizens and lawfully present residents are now required to have health insurance coverage (with some exceptions).

San Ysidro Health conducts outreach at various community locations and is also available to give presentations to organizations and community groups to provide a simple explanation of the new free and low-cost health insurance options. The Team is also available for table outreach opportunities at health fairs and events.

In addition, San Ysidro Health has a Health Insurance Hotline (619-600-3000)  that provides phone screening and health insurance enrollment appointments to those that qualify for health insurance plans with Covered California & Medi-Cal. The number is 619-600-3000 and it’s available Monday through Friday from 8:00am to 5:00 pm.

Health Promotion Outreach Services:

The Health Promotion Team provides blood pressure screenings, BMI screenings, and health educational materials on a variety of topics including: chronic disease such as diabetes, hypertension, high cholesterol, and cancer; healthy lifestyles and prevention; nutrition and physical activity. The Team provides services at clinic sites, schools, churches, community centers, grocery stores, malls, street outreach, community health fairs, etc.

HIV Outreach Services:

The HIV Department provides a comprehensive array of quality HIV services in a caring and supportive environment. Our services include, HIV testing, medical services and social support for infected and affected individuals and their families.  Outreach and HIV testing may be conducted at clinic sites, schools, churches, community centers, grocery stores, malls, street outreach, community health fairs, etc.

Dental Department Outreach Services:

The Dental Department Outreach Team provides health fair Dental Screenings for Children 1-15 years of age addressing Oral Hygiene, Fluoride and Sealants.  The Team provides services at clinic sites, schools, churches, community centers, grocery stores, malls, street outreach, community health fairs, etc.

Mobile Health Services:

The Mobile Health Services Team provides pediatric preventive health care, school and sports physicals, immunizations, WIC, Hemoglobin test, minor sick visits and some adult services, e.g. pregnancy tests, family planning and birth control. Blood Pressure and Body Mass Index Screenings are also available at health fairs and other community events.  Areas of service include shopping malls, public housing, retail stores, schools, parks and recreation centers.
